Admissions

To make a referral or for more information:
Contact our Admissions Department at (814) 878-1288 or (814) 878-1200. One of our Rehabilitation Liaisons will respond to perform a pre-admission screening and coordinate transfer to our facility.


Home Evaluation
The hospital provides no cost home screenings for individuals who might benefit from rehabilitation. To facilitate a screening, the individual, a family member or a healthcare professional should call (814) 878-1288 to request a home evaluation. A liaison will visit the individual's home, complete the screening and make appropriate recommendations.


Insurance
Rehabilitation is covered in full or part by most health insurance plans, including Medicare and Medicaid. The hospital has contracts with most major insurance companies and is willing to negotiate with other companies as needed.


Patient Admissions
When possible, we ask that patients be admitted between 8:00 a.m. and 2:00 p.m. This allows patients to become oriented to their new surroundings and meet the staff.


What to Bring on Admission Day
  • Please have a family member accompany the patient when admitted. If the family is unable to accompany the patient please contact the admissions office (814) 878-1288 or (814) 878-1343 to make other arrangements.
  • Medicare Card
  • Other Insurance Cards
  • Patient’s Drivers License
  • Medical records for the admissions office to copy (if needed)
  • Emergicare (or any other ambulance service) membership card
  • Copy of Living Will, Power of Attorney or Advance Directive (if applicable)

Discharge planning begins early in the rehabilitation process. Your case manager will guide you and your family through this process, but other team members will also be actively involved. On the day of discharge, patients usually check out of their rooms by 11 a.m.
